Open Enrollment Is November 13–28
Open Enrollment is your once-a-year opportunity to modify or enroll in the TEAM benefit plan options as well as update your dependents. This enrollment is a PASSIVE enrollment. This means your current 2023 benefit elections will automatically carry over to 2024, except for the following benefits:
- Medical – Curative will replace BlueCross BlueShield. You must enroll in a Curative plan to have medical coverage in 2024.
- Flexible Spending Account (FSA) – You must re-enroll in the Healthcare FSA each year in order to participate.
- Buy Up Short-Term Disability (STD) Insurance – This plan will require a re-enrollment for 2024.

Curative will replace all BlueCross BlueShield plans with two medical plans in 2024:
Curative EPO and PPO
The Curative Exclusive Provider Organization (EPO) and Preferred Provider Organization (PPO) have the same benefits, except out-of-network benefits are not covered under the EPO. Your coverage with Curative will be contingent upon the completion of a Baseline Visit within the first 120 days of coverage.
Baseline Visit. You will meet your designated Care Navigator and a clinician virtually, from the comfort of your home to jump-start your health and wellness journey. They will orient you to Curative, connect you to wellness programs, and answer your health-related questions. All members and dependents 18 or older in a Curative plan must complete a Baseline Visit to receive the richer benefits.
- With Baseline Visit

- No in-network deductibles, copays or coinsurance.
- Without Baseline Visit
- In-network deductibles will be $5,000 for individual and $10,000 for family.
- No charge for in-network preventive care and virtual visits.
- In-network copays will be $25 for primary care and $50 for specialty care, after the deductible.
- PPO Out-Of-Network*
- Out-of-network deductibles will be $10,000 for individual and $20,000 for family.
- No charge for out-of-network preventive care. Virtual visits are not covered.
- Out-of-network copays will be $25 for primary care and $100 for specialty care, after the deductible.
- * Out-of-network benefits are not covered under the EPO.
Go to curative.com/baseline to schedule a Baseline Visit. If you don’t complete a Baseline Visit in the first 120 days of the year, you will have higher medical costs and miss out on a $0 deductible.

Prescription Drugs
The Prescription Drug Program is coordinated through Curative and Kaiser. You may find information on benefits coverage and search for network pharmacies by logging on to Curative, kp.org for Kaiser, or by calling the Customer Care number on your ID Card. Your cost is determined by the tier assigned to the prescription drug product. Products are assigned as Generic, Formulary Brand, Non-Formulary Brand, or Specialty Drugs.

Other Benefit Changes
- Short-term disability (STD) and Long-term disability (LTD) are moving to Metlife.
- TEAM is reinstating a company-provided LTD with an LTD buy-up option through Metlife.
- A hospital indemnity plan will be offered through Metlife.
- Health Savings Account (HSA) will no longer be offered.
Costs for Coverage
We are pleased to announce there is only a slight increase to medical premiums for 2024.

How To Enroll
You’ll enroll for your 2024 benefits in Workday. On your desktop or mobile device, visit myapps.microsoft.com to locate the Workday app. Then select the Workday icon. On the “Welcome Page,” you will see the 2024 Open Enrollment announcement. Select the Benefits Open Enrollment link to start.
